Scenario
The client produces their proof of vaccination for back-entry, but the immunizer is unknown.
E.g. illegible name, signature only, box is blank, etc.
1. Open ImmsBC to see if there is an appointment for that day in vaccine_admin. Check if any partial information was saved by the immunizer (e.g. informed consent, vaccine details, deferral notes).
If the immunizer's name appears in the consent section, then it can be assumed that the same individual provided the vaccine.
2. Depending on the service delivery location, try to investigate:
- Is there a physical signature log, sign in sheet, or flow sheet?
- Run the DIWA Submissions or Daily Immunizations Report to find the names of immunizers that day. It may be possible to identify the person based on their initials, signature, or unique aspects of their name.
- Did the client attend the appointment with another person (e.g. sibling or parent)? If the other person's record is complete, it could be assumed that the immunizer is the same.
- Other: check external systems (e.g. PARIS, Care Connect) for Case Notes, discharge summaries, or any other documentation that may identify the provider.
3. Check with your clinic lead to see if there are site-specific data entry protocols.
- One clinician may be the designated "default" immunizer.
- As a workaround, the non-ImmsBC Provider (Contact) called "VCH Unknown VCH Unknown" may be selected.
